Important Note - formal reports (ie. SR-100, SR-200) must be written in professional and objective language. See this guide for more information on how to write in this style.
General forms are here for all staff and volunteers to use as needed.

Housing Request Form - this form is for staff members to request for accommodations during our physical event. The form also contains detailed information about the requirements, process, and other policies.
Formal Report Form (SR-200) for harassment, discrimination, or bullying - if you are not a supervisor, this is an optional form you can use to formally report any harassment, discrimination, or bullying (including retaliation) involving a TouhouFest staff or volunteer that you have witnessed, experienced, or heard about. This form is required for supervisors under the obligations of their role.
Staff Information Form - use this form to update your information with Staff Resources including legal name, preferred name, pronouns, time zone, and contact information. You may also be directed by Staff Resources to submit this form if we're missing anything.
Supervisors are defined as anyone that holds a position in TouhouFest ranked as a manager, director, vice chairperson, or chairperson. Supervisors also include anyone else that directly supervises other staff or volunteers.
Required Report Form (SR-100) for disciplinary action by a supervisor - Supervisors are required to submit Form SR-100 to the Staff Resources Department within 72 hours of taking a formal disciplinary action against a staff or volunteer that they supervise (including formal warning / "write ups", suspensions, and terminations). Non-disciplinary counseling/coaching and informal warnings do not require the submission of Form SR-100 (but the supervisor can choose to submit it optionally if they want it to be documented by Staff Resources).
Required Report Form (SR-200) for harassment, discrimination, or bullying - Supervisors must report any known or suspected harassment, discrimination, or bullying (involving a TouhouFest staff or volunteer) to the Staff Resources Director immediately upon being made aware. Submission of Form SR-200 counts as fulfilling this obligation.
